If your gynaecologist has recommended an NT scan, you may be wondering what it checks, whether it’s safe, and what your results actually mean. The good news is that the NT scan in pregnancy is a simple, painless ultrasound that gives you valuable early reassurance about your baby’s health. In this guide, I’ll walk you through exactly what it is, when it’s done, what the numbers mean, and why this first-trimester screening test is one of the most important checkpoints in your pregnancy journey.

What is an NT scan?

NT stands for nuchal translucency — a small, fluid-filled space at the back of your baby’s neck. The NT scan is an ultrasound that measures the thickness of this space in millimetres. Every baby has a little fluid here in early pregnancy; that’s completely normal. But when the measurement is thicker than expected for the baby’s age, it can signal a higher chance of certain conditions, which is why we measure it carefully.

Often, the scan also checks for the baby’s nasal bone, which is why you may hear it called an NT NB scan (nuchal translucency and nasal bone). Together, these markers give us an early, non-invasive window into how your baby is developing.

Why is the NT scan so important in the first trimester?

The NT scan matters because of timing. The fluid behind the baby’s neck can only be measured accurately during a narrow window in the first trimester — after that, the body gradually absorbs it and the scan becomes unreliable. This makes the NT scan one of the earliest opportunities to assess your baby’s risk of chromosomal conditions, long before later anomaly scans.

For most expectant parents, the bigger value is peace of mind. A reassuring result lets you move forward in your pregnancy with confidence. And in the small number of cases where the scan flags a higher risk, finding out early means you have time — time to ask questions, consider further testing calmly, and make informed decisions with your doctor rather than feeling rushed. Early information is empowering, and that’s the real purpose of first-trimester screening.

When is the NT scan done? (The right timing)

The NT scan must be done between 11 weeks 0 days and 13 weeks 6 days of pregnancy. The ideal time is around 12 weeks, when the baby is large enough to measure precisely but the nuchal fluid is still clearly visible.

I always encourage my patients to book this scan in advance, because the window is firm — there’s no extending it by a few days. If you’re unsure how far along you are, your last menstrual period (LMP) or an early dating scan will help us schedule it correctly. Missing this window doesn’t leave you without options (more on that below), but the NT scan itself genuinely cannot be repeated later.

What does the NT scan detect?

The NT scan is primarily a screening test for chromosomal conditions, and it also offers a few other early checks. It helps assess the risk of:

  • Down syndrome (Trisomy 21) — the condition most commonly associated with this scan
  • Edwards syndrome (Trisomy 18) and Patau syndrome (Trisomy 13) — rarer, more serious chromosomal conditions
  • Turner syndrome and certain other chromosomal differences
  • Congenital heart defects — an increased measurement can sometimes be an early clue, prompting a closer look at the baby’s heart later

Beyond these, the scan also helps confirm your due date, check whether you’re carrying twins or multiples, and see that the baby’s heartbeat and early growth are on track. So while it’s best known as a Down syndrome screening, it tells us a good deal more about your baby’s early development.

NT scan normal range: what the measurement means

A common question is, “What is a normal NT measurement?” As a general guide, a thickness of under about 3.0 mm around 12 weeks is considered within the normal range. However — and this is important — the exact cut-off depends on your baby’s precise gestational age (measured as the crown-rump length), so the number is always interpreted in context, not in isolation.

It’s also essential to understand that the NT scan gives a risk estimate, not a diagnosis. Results are usually expressed as a ratio, such as “1 in 1,500” (a low-risk, reassuring result) or “1 in 50” (a higher-risk result that we’d want to look into further). A low-risk result is very reassuring, while a higher-risk result simply means further conversation and possibly more testing — never a definite answer on its own.

What if the NT measurement is high?

If the nuchal translucency is thicker than expected — generally above 3.0 to 3.5 mm — please try not to panic. An increased NT measurement raises the probability of a chromosomal or heart condition, but it does not confirm one. Many babies with a higher reading go on to be perfectly healthy.

What it does mean is that we’ll talk through your options together. These may include a NIPT blood test, or diagnostic tests such as chorionic villus sampling (CVS) or amniocentesis that can give a definitive answer, and sometimes a detailed fetal echocardiogram to examine the heart. NT scan + blood test: the combined first-trimester screening

The NT scan is far more accurate when paired with a simple blood test — this is called the combined first-trimester screening, or double marker test. The blood test measures two markers produced during pregnancy: free beta-hCG and PAPP-A (pregnancy-associated plasma protein-A).

By combining the nuchal measurement, your blood results, and your age, we calculate a much more precise risk score. On its own, the NT scan detects roughly 70–75% of Down syndrome cases; combined with the blood test, the detection rate rises to around 85–90%, with fewer false alarms. That reduction in unnecessary anxiety and invasive follow-up is exactly why the combined screen is the modern standard of care.

NT scan vs NIPT: how do they differ?

You may have heard about NIPT (non-invasive prenatal testing) and wondered how it compares. NIPT analyses fragments of the baby’s DNA in your blood and is highly accurate — over 99% — for the common trisomies. The NT scan, on the other hand, is an ultrasound that, beyond chromosomal risk, also gives anatomical clues (like early heart markers) that a blood test can’t.

The two aren’t rivals; they complement each other. Many of my patients have the NT scan with the double marker test first, and NIPT is recommended when there’s a higher risk or for added reassurance. We’ll decide together what’s most appropriate for your pregnancy.

How to prepare for your NT scan

Preparation is easy. For an abdominal ultrasound, you’ll usually be asked to arrive with a moderately full bladder — drinking two to three glasses of water about an hour before helps give a clearer image. Wear comfortable clothing, and bring along your previous reports and LMP details. Occasionally a transvaginal scan gives a better view, in which case a full bladder isn’t needed.

The scan is completely safe for you and your baby. It uses sound waves, not radiation, is painless, and takes around 20 to 30 minutes.

A note on the NT scan and your baby’s gender

To be clear: the NT scan cannot and does not determine your baby’s sex. It is purely a health-screening test.
